Enhanced Supportive Living/Medically Fragile Residential Program

Carey Counseling Center Inc.’s Enhanced Supportive Living/Medically Fragile Residential program is a holistic program that ensures a safe environment for those with severe, persistent mental illness along with co-morbid medical issues. These facilities exist to provide and/or facilitate housing, nutrition, mental health, and medical service for adult individuals with severe and persistent mental illness. The supportive housing facilities are staffed twenty-four hours a day, seven days a week.

Our residential program offers a step-up incentive to its members. After a member has completed their goals at the Enhanced Supportive Living/Medically Fragile facility, they can then step up to the Standard Supportive Living Housing Facility program and eventually progress to one of CAREY’s Independent Living Apartments or Houses. With this step-up program, a safety net is also provided with continuing case management contacts. If it is determined that a member is not coping with a more independent level of care, they can return to the more controlled environments of the enhanced or standard supportive housing living facilities.

Carey Counseling Center Inc.’s Enhanced Supportive Living/Medically Fragile Residential program includes two coed facilities:

  • Herrington Place (offering 27 beds)
  • Poplar Bend (offering 15 beds)

Carey Counseling Center, Inc.’s Enhanced Supportive Housing Program includes two coed facilities:

  • Camden Group Home: (offering 9 beds with one additional bed dedicated to crisis respite services)
  • Trenton Group Home: (offering 9 beds with one additional bed dedicated to crisis respite services)
  • + Upon admission to the program:

    + Upon admission to the program:

    • Members will be provided with extensive case management, at a minimum of three times a week.
    • Members will receive an intake interview with a licensed clinical social worker (LCSW).
    • Members will be evaluated and have an admission completed by LPN nursing staff.
    • Members will receive an initial psychological evaluation performed by the director of mental health services or appropriate staff within seven days of arrival.
    • Members will receive a physical examination performed by a licensed primary care physician within seven days of arrival.

    • The individual must be eighteen years of age or older.
    • The individual must have a co-morbid medical diagnosis.
    • The individual must be sufficiently alert to engage in placement planning done on his/her behalf by others (e.g. family, friends, mental health agency, hospital, etc.).
    • The individual must be able to perform basic self-help activities including but not limited to eating, bathing, dressing, transferring, and toileting.
    • The individual must be able to care for their own possessions and to maintain their bedroom (living area) in a reasonable state of orderliness and cleanliness.
    • The individual must be able to recognize danger or threat to personal safety.
    • The individual must be able to live comfortably within any limitations in the structure of the facility.
    • The individual must be generally able to maintain appropriate behaviors tolerable to the community.
    • The individual must be able to acknowledge safety hazards and comply with facility guidelines (e.g. fire alarm, tornado warning, etc.).
    • The individual must receive disability below the maximum allowed.
    • The individual must receive Medicaid/TennCare benefits.
    • The individual must have a background check completed.
  • + Program Exclusion Criteria:

    • The individual cannot be on the sex offender registry.
    • The individual must not be on mandatory outpatient treatment (MOT) as ordered by the judicial system.
    • The individual must not have any pending legal charges.
    • The individual must not have any forensic charges.
    • The individual can be safely maintained and effectively treated at a less intensive facility.
